Below are the Medical History and Privacy Policy forms you’ll need to complete on your child’s first visit to our office. You can expedite matters by downloading these forms, completing them at home, and handing them to our staff when you arrive for your initial visit to our office.

What to Expect at Your First Visit
On your child’s first visit, we will review their medical and dental histories and evaluate their oral health. We will take digital x-rays of your child’s teeth and explain whether or not they will require any dental treatment. We will be more than happy to answer any questions you might have.
